    I believe that more women grassroots' groups should be encouraged and trained as trauma recovery, healing and management counsellors, their "tender care giving" will be an avenue to support and link People living with HIV and AIDS; PLWHAs will be stabilized, youths will be more receiptive to Global Fund & country Sensitization and Awareness raising efforts about the pandemics. 
     
    Much is being done but Boys and Girls need to be contineously infomed.  You are right, that "NGO's that have as main goal to improve the condition of all should be supported".   In my country, it is the grassroots community based organizations (CBO's) and local NGO's that toil to reach out to help inform and change society.  Most of these groups struggle to fundraise so that they can support their communities and groups.  
     
    Capacity building for most of these women's groups will open the gateway for networking, combat the stigma and fear people die of within countries and families.
     
    Education and human rights interventions particularly focusing harmful traditional practices against the girl child, will reduce stress and encourage all to participate equally in dignity.

    Considering the theme of the topic one should know what the needs of women are. It is fact that human being is the most beautiful creature of the nature. Human being has got the power to thank to decide and got sexual power, which consider a gift from the heaven. According to the Islamic point of view if sexual emotions were fulfilled as per the law of God it does not make any difference and it is considered the beauties of world. So it is not possible with out a woman. Our Holy Quran tells us that wives have beautified the world and familiarized the people with love. God created Adam and for his companionship "HWA" was created. Women are the means to make the humanity continued up.
          
    When a woman is so ends pensible, she has the some rights and needs as well,

    1. Her sexual emotions be respected legally and as per the Islamic point of view.

    2. She should be given full right in the society.

    3. She should be consider a dignified persons.

    4. Women have got more deficiencies comparatively in male, so these should be removed.

    5. They hate harsh treatment and disregard.

    6. Women and good food and good attire.

    7. Women need good education and dream of shining future.

    8. Women need sound health and proper care.
     
    9. These are some of the needs of the women for stage of minority. Slowly and gradually they try to make this mental picture of the future to be materialized.
         
    In various societies women are treated very harshly even in some backward areas of the
          
    Muslims countries. The best of girls is considered to be a burden on family and a sign of disgraceful but frankly speaking it is a sign of joy because the women is the most beautiful creature. In some societies women are sold and are treated as animal are treated in an exhibition-come-market and eventually women change their nature and become rebellious. It has been noticed that they have defamed very famous and notables of the world. Consequently such women fall into the hands of such notorious persons who destroy their lives and eventually these women suffer HIV, Aids Malaria and tuberculosis.

    In certain Male issue is known as gift and a guarantee for a future of a family and as such when a baby is born no proper attention is given neither to its health now to her education. From the very early childhood she suffer the inferiority complex. Later on when she grows, misfortune accompanies and the last she suffers in various disease mainly tuberculosis.

    In any opinion of the following means are adopted so many short comings, difficulties and diseases be avoided or at least be minimized.

    1. Co-education on secondary level and high level be discouraged. when ever matured  male and female study or read collectively, naturally emotion , arise and leads to unnatured and illegal activities. That is why Islam for beds co-education and even it is said that keep your eyes down and such action is impossible in collective education.

    2. Islamic point of view no nature girl is allowed even a women is not allowed go to out side their home. May some bad eyes follow and destroy her future. May the man be an HIV, AIDS Stricken. so no minor or elder be allowed to go alone.

    3. we see various kinds of literature, which consists nakedness sentimental and insipidly pretty, which are published just for amusement but it put an adverse. It leads not only male but female as well to a disastrous situation. To control sexual emotions both the sex adept every means. Male becomes a deceit  a murderer and abductor. The opposite six awes not know who the male is and what kind of disease he is suffering from consequently the entire society is spoiled. These should be banned.

    4. in some countries blue prints are prepared and circulated through  out the world which have very bed effects. This blue print are witnessed by all the family members which is consisted some minors as well. The enjoy it but the condition of the minor worsens, when a kid asks his father or mother,, what is there going on ,, this also leads the minorities to get maturity before time. The result is facing various epidemic diseases and lost of precious lives.
     
    5. though ‘’ perda’’ (hiding face with a cloth)is considered not good in so many societies but hiding face is not only the real thing the entire body should be covered so that eye may not fall on.

    6. in so many civilized countries a big ratio of population is addicted. They Smokes drink, eat inject and use drugs. It is increasing by leaps and bounds. These drugs are the first stages of destruction, immorality and diseases. it is becoming very common because the world is going to become a billions in a day. This drug abuse needs a strong wall to be baned.

    7. Islamic point of view every male and female be get married as and when he reaches adolescence . in case more time is given more badness creates. So this should be given priority.

    8. every learned man i.e. Doctor , philosopher teacher, malvi, clergyman and leaders of a country preach against the evil of HIV, HIDS and so many other men ethical actions. They themselves also avoid such activities.

    9. keeping beard is now a days considered to be a sign of terrorist but I think these people are free of such disease, because they are free of the germs which are expected to be dangerous for a clean shaved person.

    10. Poverty I consider the most sensitive aspect of this disease. We see such diseases very common in some poor countries. In case we want the remedy of these diseases we should eliminate poverty. Well to do countries should help the poor. They should create job opportunities and make education common.                  

    These are the mane and burning items which are sound in approach, feasible and has the potential for sustainability and impact.

    About 80% of HIV infections are contacted through sexual intercourse. There is need for value re-orientation i.e. information, awareness and sensitization. Sensitization to inform the people and effect behavioural change through Training Workshops of selected opinion movers that influence Communities, Seminars and Symposium. The people will have to be informed in order to access Health Care facilities.

    Condom distribution, Accessibility of ARV, encourage Training on HCT/R.

    If you are not informed, you are misinformed, when you are misinformed, you are deformed and a deformed person cannot perform. For people to demand for access to health care delivery services there is need to eliminate all weaknesses and constraints in health systems i.e. ignorance “information and communication”, certain cultural beliefs and practices, manpower.

    Background:

    Over the past 2 decades, HIV/AIDS/TB has emerged as one of the biggest killer disease of human kind.  The disease has crossed the 40 million mark of those infected, leaving millions dead.  This disease has not only devasted the lives of those infected but also their families, communities and the conomics of the countries.

    90% of the working force engages in unorganised labour sector nearly 50% of the population are illiterate.  The young girls and boys aged between 13 and 17 years are also at risk.  Treatment seeking behaviour is lacking and getting drugs across the counter is high.  They have greater opportunities to involve in high risk behaviour activities irrespective of their age and sex.

    Nearly 40% of the married women and men have multipartner sexual behaviour.  Some married women often agreed to have sex with their casual partners to get personel favours fulfilled.  A large number of unmarried girls had premarital sexual relationship with their boyfriends/relatives/neighbours/coworkers.  About 50% of unmaried youth had sexual relationship with married women. 

    Because of these above factors, the working force women, and girls are infected by HIV/AIDS/STDs/TB and they unknowingly spread to others.  To eliminate this situation awareness generation on HIV/AIDS/TB/Malaria and establish a counseling centre to the same.


    Approach and Strategies:

    1. Development of core groups of girls and women labourer by imparting training and knowledge on HIV/AIDS/TB/Malaria for using them as peer educators.

    2. Conducting awareness generation campaigns for the workers at their work place. 

    3. Medical and paramedical workers shall be given trainign on safe injecting practices.

    4. The local community leaders shall be approached for advocacy and social mobilization.

    5. Setting up of Health care centre for clinical services.

    6. Setting up of counselling centre for motivating individuals to attend clinics for HIV/AIDS/TB testing if required.

    7. Condom distribution through free outlets & social marketing.

    8. Use of folk media groups for communicating HIV/AIDS/TB/Malaria problems to the community.

    9. Supply of IEC materials and medicines to the needy persons.


    Outcome:

    1. Increased level of knowledge on HIV/AIDS/TB/Malaria

    2. Level of knowledge among risk behaviour groups and general community.

    3. Reduced fear about HIV/AIDS/TB

    4. Increase in number of safer sexual behaviour

    5. Behaviour change communication will be increased.

    6. Distribution of IEC materials and medicines will prolong the life time of the infected persons.

    Awareness generation by NGOs for voluntary Blood test by every one in the community and no disclosure of the HIV/AIDS affected so that the stigma is not attached to the sexual minoroties so affected.

    HIV is a fatal disease which has been sent to Afghans from abroad fortunately it has the lowest numbers in Afghanistan the best way to avoid it is to aware the people from it.

    The how to prevent it:

    - People must use condom and we must aware people from it's profits (with out pregnancy plan) that is the best way to avoid HIV and avoids serial births

    - To make special hopital as center for treatment not as insult.

    - To make people aware of the Islamic way in husband and wife lives and avoiding illegal people

    - Avoiding sexual affairs with animals.

    - We must aware people by the teachers, mosques, clinics, and the best way of advertismets must be done by the women.

    According to The Global  Fund it must visit people in their sites. When you talk to beneficiaries themselves will be better I think that they need some thing to do in order to do generative incomes. The people whom we need are those who are not effected too much, so we need to do Mobilization in country side  not in town because there there is no development so lying illustrate peoples is very easy so we need  to visit  those  targets people and the poverty is one thing about the HIV/AIDS.

    You may focus on vulnerable peoples.

    HIV Aids today can described as a moral disease where in solutions lies with ability for all to control moral misbehaviours.

    As long as men and women reproduces beautiful one will continue to come, but it must clearly said that the West should stop ladies from going back to barbaric ages where women are used and dumped.

    I want explanation from men who put on thier underwear, shirt,ties and thereafter put a coat to look dignified will always allow his "only wife" thai is for him only to expose all or part of his beauty to other men to see. This is a moral question.

    To ladies, why will beautiful women be used to entertain guests (some of those whose heart have serious within them each time the see women) who will then turn back to all cost practice what they saw immediately.

    Then who is fooling who? International organizaion and developed countries are making thier monies as long as drugs are produced, requested  for and sold, who will stop a business men and women from making gains out thier sales? HIV/Aids question is a moral question.

    Finally until a president of a country is declared to have died of this Aids,that a prime minister has been infected the world will continue this way.

    -Responding to the needs and rights of especially and included and neglected groups, such as women and girls living in poverty and extreme poverty, adolescent working with men and boys, migrant women workers, domestic workers, HIV positive women or on especially neglected forms of violence or issues, such as sexual violence against girls and young women, abuse during pregnancy, political or economic violence, links with HIV and AIDS, and sexual violence in conflict and post-conflict situation should be perfectly addressed.

    -More research is needed both at national, sub-national, local and community levels for better understanding of HIV/AIDS, tuberculosis and malaria.

    -Providing survivors with quality health and other supportive services.

    -Human rights-based and gender-responsive approches that place paramount priority on promoting, protecting and fulfilling the human rights of women and girls,as well as strengthening institutional capacities at local and national levels to eliminate all forms of violence against women and girls.This includes addressing inequitable gender norms and power disparities as the root cause  of violence against women and girls, and as violation of human rights and an impediment to development.

    -In order to achieve all these, all human race should be ready to promote and protect human rights.

    -  Prevention  through adolescent education on sexual and reproductive health  with a focus on gender which can be provided at schools,  through media  or by affiliated religious  bodies to prevent acquisition or further transmission of HIV .

    - Training programs on primary health capacity  building among First Level Facility Health workers ,  Primary Health  Centers , Family Planing Centers and District Outpatient Clinics to support primary prevention of HIV and other Sexually Transmitted Diseases among adults and women of reproductive age that  include reduction of unintended pregnancy among HIV positive women , prevention of  mother to child transmission and to provide care to support HIV infected women. And  extend activities to increase awareness  to include vulnerable people e.g Drug addicts , Sex workers , Truck drivers , Prisoners, adults with STDs , School dropouts and Street Children.

    - High priority should be given to scaling up HIV and other Sexually Transmitted Diseases testing  and counseling to maximize opportunities to reach those with HIV and expand testing in antenatal areas , family planning clinics  , sexually transmitted infection clinics in patients wards  and to support Researches related to these issues.

    -Supporting NGO's that have as its main goal to improve conditions of the women and street children

    It reveals from the critical review and analysis of the existing health services in Bangladesh  that less than 40 percent of the population has access to basic health care and more than 70% pregnant women do not receive maternal care or assistance from a trained attendant. While the services are inaccessible to the most vulnerable groups the women, children and poor in particular, the government health services are left unutilized and under utilized. While some of the services are open to selective classes of people, the quality of services is unacceptable. Comparing the use of different levels of public health care delivery facilities by income groups shows the richest 20% of population are much more likely to use hospital services than the poorest. 

    In addition, there are emerging health needs such as HIV/AIDS when Bangladesh is considered as a potentially vulnerable country due to its geographical location close to high epidemic countries. Due to presence of vulnerability factors such as growing number of sex workers and intravenous drug users, high rates of STDs and RTIs etc. Bangladesh is likely to face serious health hazards in coming days. While the rates of Tuberculosis is already high in Bangladesh, if HIV/AIDS begins to pose threats, it is extremely likely that there will be a parallel epidemic of HIV/AIDS and Tuberculosis as the former affects the immune system of the body. This is an overall scenario of the country.

    Urban and rural poor in Bangladesh especially those living in the 05 Upazilas in Barisal District   suffer from the worst health status in the country, and the situation will continue to worsen because of the influx of the rural and urban poor in  causing rapid and unplanned population growth. Most of the poor live in unhygienic rural huts like  with very poor living conditions. Insufficient GO/NGO health service centers, unavailability of low cost health services beyond traditional working hours, harassment through repeated visits and unnecessary investigation are the problems that the urban and rural dwellers are facing in present health care system. Pricing of health services in the private and profit making health outlets are out of reach of common people.    

    Like other urban and rural areas of Bangladesh, Barisal is also one of the most polluted district. In the rural areas poor infra-structure facilities, unplanned using of chemical fertilizers in the crop fields, using of poor water and sanitation facilities, and in the urban areas air pollution, industrial pollution, smokes from increased number of vehicles, smokes from burning brick fields, improper use of polythene and plastic and inefficient use of waste and disposal are making unsafe environment for human beings. 

    Effective PHC system can help improve the health status of the urban and rural dwellers. Unfortunately, the urban PHC system has been given little attention so far by the Government and has received little support from external aid agencies. Although some efforts have been underway as a result of interventions under various health services in some areas of Barisal  District, even then the existing primary health care system  is yet to be developed to meet the growing health demands of its entire population. Failure to improve urban PHC will have serious negative consequences on the morbidity & mortality of the city dwellers especially the poor, women and children. 

    From the general description of the project 5 Upazilas areas and the target population therein, it is quite evident that the target populations have their unique characteristics in terms of health problems and accessibility to health services owing to their geographical settings as well as their socio-economic conditions.

    The low-income groups of the area are vulnerable in terms of health consciousness and practices mainly mother and child. People from the adjoining areas converging health culture and beliefs have developed a heterogeneous and conflicting health care culture. Such cultural conflicts sometimes restrict them to visit health facilities of the target areas. As for health measures, most of the people go for traditional and unscientific care practices that often cause health hazards. The women and children are the victims of such beliefs and practice. In such a situation there is indeed a great need for vigorous health campaign in the area.

    The effective implementation of the above would not only contribute to the reduction of mortality, morbidity and fertility but also to an improvement of the quality of life and reduction of poverty in general. The resource constraints for meeting future demand for health and family planning services that will require improved efficiency and expanded cost recovery in the health sector. Attention should however be given to the equity aspect in cost recovery, particularly in respect of women, children and the poor.

    The proposed project intends to address three basic issues, which are: a) effectiveness b) efficiency and c) equity. The project will achieve effectiveness of its program by complying with the primary health care services; improve efficiency of services by improving cost-recovery policy and achieving quality of care indicators; and improve equity by increasing access, both geographical and economic, to essential health care services for the poor and hard to reach groups, the women and children in particular. These sections of the population have been classified from information collected from secondary sources and to be validated through a household survey during first quarter of the project period. Cost-recovery has been an important agenda  to establish client's rights to receive quality services. The proposed project will recover substantial amount of its recurrent costs through pricing of services.

    Dear all,

    Sorry for delay in sharing ideas and thoughts regarding this important topic. We always in our region emphasised on the importance of raising awareness of girls and women on sexual matters, they have the right to be informed about their rights. Empowerment of women and girls is essential in all efforts to achieve the aim of HIV/AIDS prevention and control in our region.

    Through educational  activities conducted in my country ( Jordan )  we worked with leading NGOs as well as other sectors such as Ministries of Social affairs, Labour, tourism , youth, education, interior , universities and syndicates, Princess Basma Women?s ResourceCentre , Family planning association, association of Jordanian women, and association for protection of families and  NGOs working in the fields of vulnerability, , we emphasised on  vulnerability among young women which are considered as main objective in our efforts in the field of HIVAIDS. Women and girls still need more education and social support

     Vulnerably of women and girls is increased with Limited availability of prevention services in majority of countries of the region. This includes lack of access to appropriate counselling and care for STIs and voluntary counselling and testing of HIV. For young female population, in particular, the possibilities of accessing information, counselling and services on issues related to sexual health remain limited in both school and non-school settings. Efforts should be strengthened t enable women and girls especially in this region where migration, mobility and conflicts are prevailing

    The results of a knowledge, attitudes, beliefs and practices (KABP) surveys carried out in the general population in countries of the region confirm that sexual contacts do occur outside marriage in certain layers of the population .. At the same time, the use of condoms among those reporting sexual contacts was reported among less than 50%. This evidence by it self is very important to raise awareness of female population in the field of HIV/AIDS, and to   give them the power how to protect themselves. They should be educated and empowered

    1.  What are the most effective health and social interventions that link HIV/AIDS and sexual and reproductive health?

    The most effective health and social interventions Training programme on primary health care through capacity building of stake holders especially Panchayati Raj Institutions and women and adolescent girls may be supported by Global Fund (RCH) to NGOs.

    Involve NGOs in activities like RCH, sexual education to dropouts and non school going youth.

    2  Which ones should be supported by the Global Fund and which ones should not?
