Dear Members,
In the event to develop capacity the countries to prepare the proposals of the projects and to implement programmes successfully, we can point out the difficulties in this regard.
The critical system of project proposal of the Round by the GlobalFund be simplified.
Capacity development is to develop abilities of individuals, institutions and civil societies to perform functions, solve problems and set and achieve objectives in a sustainable manner. Capacity Development is the process through which these abilities are obtained, strengthened, adapted and maintained and managed to combat the evils of pandemic of HIV/AIDS, TB, and Malaria.
Capacity Development process involves education, training and knowledge services. Improvement of behavior, knowledge, understanding, attitude, values or skills are key instruments. GF may make the teams well conversant and involve in the capacity building for the participant countries in regular manner.

In order for countries to that receive the capacity development support they need for the preparation of proposals and the implementation of programs, global fund should work with think tanks that involved in training. For example, some proposals would be developed with the think tanks; these may also help in implementation.
Toolkits for proposal development should also be put in place. Countries should also be encouraged to make their national implementation plan. The funding guidelines should indicate the priority areas that need urgent funding and attention.

Dear e-Forum,
Gf should shift its focus from project management and implementation to capacity development; limiting itself to local governments, natural resources management, and private sector development which should include gender consideration, provision of technical support, advocacy and strategic planning.
Organisational and technical capacity building is a cornerstone to GF initiative approach and will help ensure that countries have the necessary skills and strategies they need to prepare and implement effective HIV/AIDS programmes that pertains prevention and transmission of HIV and related issues. This should focus on organisational effectiviness such as; planning and management, trust building, organisational self assessment solution formulationand capacity development activities as well as technical HIV issues including behavioural change communication, monitoring and evaluation, micro-credit/finance, gender issues, partnering and advocacy, stigma, sexual coercion, cross generational relationships and transactional sex in order to achieve maximum behavioural objectives and reduce stigma.
GF should develop a tool/martix to enable countries identify, analyse levels of capacity in different organisational and technical support needed by each organisation in the country and moinitor and eveluate the impact of capacity building support. This should be designed to facilitate group discussions within recipients and external facilitator providing capacity building support. The tool should be self administered by each country to help them describe overall goals, objectives and maintain activities per component including expected results and timeframe for achieving the results. There is also need to balance individual rights with social responsibilities since some cultures, value family and community. These values must be balanced with the needs and desires of the individuals for effective project implementation.
GF should establish technical support units in each country to complement GF's efforts to provide guidance and support to countries so as to facilitate HIV prevention. Technical support in the form of advice to sub-recipients including civil society organisations thet receive budget support from GF in the areas of baseline data, and MIS, participatory and planning, quality assurance, coordination of capacity building, inter and intra country sub recipients co-operation, reporting and accountability, evaluation and monitoring as well as providing specialised technical assistance.
One of the greatest challanges in project implementation has been lack of capacity to manage Gf programmes as well as sustainability. In many countries CSOs/NGOs need strong support for organisational, institutional and technical capacity development for effective participation in service delivery. Government forms the principle player in management of GF programme. In line with their policies, resources from the centre are channelled through lower level governments. GF demands increased capacity for lower level government to effect new roles of integrating community plans, resources mobilisation, management contract, quality assurance and ensuring value for money, reporting and capacity development of lower local governments to effectively play their roles and responsibilities. Technical support and capacity building activities at this level will address among others ;integreated planning and coordination,institutional reform, private sector participation,information and awareness raising, coordination and collaboration of major actors, overall sector wide approach and planning and sustainability of multi sectoral components of GF, financial management.

Regarding partnership ,evaluation of health assistance resources,local technical resources,&country's technical assistance including availability of testing methodlogy, laboratory facility, national notification system,paramedical resources with supervision,knowledge, skills & their coverage could give information,identification of technical assistance needs .
Above evaluation &its integration of technical assistance budget, could ensure better use of local resources, measuring quality of technical assistance ,adjustment of technical priority, technical priority,technical capacity strengthening to local needs. From GF framework document standpoint TRP,TERG,an independent evaluation component could give technical assistance partnership, strategies by providing PR to spend technical budget in cost - effectice approaches& demand orientation,more awareness of technical standards&integration with local resources in less GDP countries.
On the basis of above technical approaches, GF technical components could give technical partner environment to non-technical, less GDP countries.

Ensuring GF impact on Capacity Development:
-
Funding CBOs, FBOs, directly without involvement of NGO’s on capacity development of programs, implementation known for using funds for themselves with little involvement of communities
- Deployment of Technical assistance in capacity building, planning and M& E of programs
- Concrete assessment on impact of resources in reducing the level of diseases prevalence in the countries funded
- Quality of proposals from CBOs, FOBs should indicate the levels of their skills in preparations of proposals
- Sustained programs implementation through predictable funding
- Ensure funds are not misplaced from the intended programs as indicated in the proposal forms at country levels
- Unrepeated proposals for programs will clearly indicated the effectiveness of disbursed funds

Regarding information on available technical assistance,identification of technical assistance ,adjustment & quality, these technical implementation might differ in many developing countries having different standards of technical approaches. Degree of technical assistance ,technical standards, counselling,coverage of laboratories facility,procedures could determine identification of technical assistance in countries e.g.Application of imaging technology,conventional tomography, chemotherapy,clinical trial groups,application of recombinant DNA technology,genetic counselling,technical standards for bio- safety,limited laboratory facilities,availability of these technical approaches could give better use of local resources,adjustment of technical assistance& projection of technical assistance in country proposals development.

Planning and budgeting for the development of the proposals and identifying the TA needed is crucial. CCM members (international partners) have to support the development of national capacities needed for the preparation of the proposal. Countries and its international partners should encourage the key staff from the ministry, PR and SRs to participate in short training courses on the writing skills of proposal development targeting international donors. Additional funding from the international partners and pooling of resources would assist the countries to build these capacities. International trainings institutions, universities and International consultancy firms has to be encouraged to be GF Friends and provide training opportunities for poor countries receiving GF grant to build the human capacities of the countries.
Regionally, encourage countries to conduct a regional meeting to disseminates lessons learnt of project’s implementation. The Involvement of decision makers in writing the proposal would pave the way for a proper implementations later on. For a proper implementation, the PR assessment, SRs assessment must be conducted prior to the development of the proposal using the GF assessment tools. Gaps identified regarding PR or SRs capacities has to be reflected in the proposal. Solutions to overcome these gaps/obstacles for proper implementation of the programs has to be suggested.

Intensive Grass root Capacity building of CSOs and involvement of CSOs in preparation of country proposal and disbursement of fund since they are at the grass root and better informed.
The use of local technical assistance in order to assist CSOs to maintain a growth rate.
Sustained renewal of Community Based activities for sustainability and information.

Dear Clair M,
You have raised an important point. Lets look at the Kenya situation which I more conversant with since I have spend my two last years mostly in Kenya. One private Hospice Care Provider is in Nairobi and another one in Eldoret. Both are major cities not accessible to the rural communities. Besides those organizations are struggling to raise funds to remain afloat and the demands on them are enormous. I did meet a medical doctor that runs the Nairobi organization and found out first hand what the challenges are.
I'm a trained hospice nurse specialized in high tech pain management (hand held computers delivering narcotics) and also worked as a preceptor for hospice to orient new service providers eg. nurses in different categories. I understand that hospice care in itself isnt widely known by the organizations/CBO's that really need to have access to it.
In all fairness to Hospice Institutions...if CCMs want to consult with them regarding pallative services to be included in a proposal, Hospices should be paid for providing such services and should not be taken for a free ride. This is unethical that such services are abused.
"In addition, many palliative care providers are grass roots organisations that have limited capacity to engage with CCM members as they at full capacity delivering services. When they are in a position to apply for funding they struggle to find technical assistance in a) preparing a global fund proposal and b) getting technical assistance from palliative care consultants. It would be useful if the Global Fund could disemminate information on where CCMS and others can access technical assistance for proposals on interventions such as palliative care, e.g. through international palliative care organisations and increased support of international technical assistance agencies in particular interventions". My comments to this is that we have to understand the realities and not abuse/take advantage of providers whether local or international, but pay them for providing a so needed specialty service for the grassroots organizations. We know that the grassroots organizations are at the forefront of palliative care...Who trains them? And who is supposed to pay for their training? Who compensates Hospice organizations? Do you think they can continue providing free services when they have children, parents and the extended family to feed, pay schoolfees and transport and medical bills? The grassroots organizations lack that internal capacity and don't have the voice to express their needs.
I just wanted to know and please don't perceive me as being rude. Is proposal review a paid service from the Global Fund? Do you get paid for reviewing proposals from the GF or are you giving a charitable service to the GF?

The Global Fund as the key supporter in the promotion of education on HIV AIDS, Malaria and Tuberculosis. To ensure that funds disburse for the development and implementation of programs funded by the global fund reaches the appropriate recipient is by establishing offices in the various regions in the beneficial countries. The aim is to monitor the activities of the stake holders. The recipient should work under the supervision of the Global Fund. Beneficial should submit their monthly report to the regional office together with the video coverage of their activities. Financial statement should be prepared quarterly and audited by the GF

But with not undermining the talent, intellectuals and professionals whom are participating and taking parts or the most important parts of projects while GF is paying consultants whom are getting and generating ideas from us in this platform. You should promise payment and will give to you knowledge. Otherwise no. and good bye...

I have been working on the experience of hospice and palliative care services in accessing Global Fund funding. Technical assistance seems to be one the major barriers to this. This is in two forms.
Firstly, the proposals that I have read from CCMs have included extremely mixed understanding of what palliative care actually is - the description of palliative care can ranges from home-based care, to psychosocial support, to treatment of OIs, to support of OVCs. Each of these may be components of palliative care but they alone are not palliative care. Despite reference to palliative care in proposals it is sometimes unclear how and where palliative care is actually being delivered. For this reason, better technical assistance is needed for the CCM on palliative care as it is unclear where, if any, guidance is coming from at the moment.
In addition, many palliative care providers are grass-roots organisations that have limited capacity to engage with CCM members as they are at full capacity delivering services. When they are in a position to apply for funding they struggle to find technical assistance in a) preparing a global fund proposal and b) getting technical assistance from palliative care consultants.
It would be useful if the Global Fund could disemminate information on where CCMs and others can access technical assistance for proposals on interventions such as palliative care, e.g. through international palliative care organisations and increased support of international technical assistance agencies in particular interventions.

In preparation of country proposals & implementation of program
GF capacity development support component should involve national
health system including public & private ,independent evaluation.
Representation of above sectors might differ in evaluation,standards
& statistical approaches.In developing countries ,evaluation degree
of health services research approaches including bio-medical research,
intersectoral research,could be determining factors of capacity development support of technical assistance.
In GF framework document,GF principles have included country's
proposals for international health financing,high quality proposals
formation is not possible in less GNP ,less awareness of technology,
less resources,countries in implementation of GF program.
For proposals development technical assistance could give technical
priority associated with available resources.Technical assistance
might differ & more detailed in access of health services across
the social.economical,& geographical expanse of many developing
countries.Possible obstacles in the form of (lack of quantitative-
methods) including cost-effective models ,lack of network& system
analysis e.g. GF program evaluation& review technique could expand
needs of sub-recipient technical assistance.
In the above condition TERG(Technical evaluation reference group)
can expand its own role by providing testing methodlogy,measurement
for quality assurance,network analysis. For technical capacity
strengthening I have sent number of country's proposals since
2003 to WHO& GlobalFund.

-Non-tuberculous mycobecteria
-screening
List of proposals & mechanism 2004,2005,2006,2007,2008,have been
submitted to GF forum& proposal team.
These proposals were based upon analysis of highly critical factors
for capacity development & analysis of possible obstacles in preparation of country's proposals.
These proposals have given better predictability of GF partnership
strategies& formal relationship to affected population in South-East
Asia.
