The Globlal Fund should divesify its funding donors, beyond governments. But it should establish clear criteria for selecting these donations, based on strict ethical considerations. Donations should come with no strings attached. At the same time, The Global Fund should not accept in-kind donations, as this is not part of its core fucntions: the Fund is a financial institution, with no logisitics or distribution capacities. In kind donations should be left for development organizations, including UN agencies and civil society organizations.

It is clear that if GF objectives are to be achieved, organisations beyond donor governments that contribute financial and other resources to the GF be an integral part of activities. Earlier I had suggested the need to target resource mobilisation. This should be complemented by guidelines which define what role each organisation will be required to play in their countribution. Individual preference of organisation would better harmonised if the GF funding requirements are presented as one proposal which should highlight priority areas and coordinated by defined bodies.
 
At present it seems this activity is poorly coordinated and optimised for the common good. As a fundamental activity, the requirements for these organisations should include:
a) All approved projects for Gf be prioritised accross all sectors to ensure that they form part of a single proposal document. In addition, funding should be targeted accross all sectors to ensure that competing sectoral demend are harmonised and their resources are equitably allocated;
 
b) These organisations should identify their sectoral interests that will be used as a guide in targeting their support;
 
c) A standard format for documenting all funding pledged should be developed and applied by all partners;
 
d) All Pledges must be categorised and specific(Not pledged, Partially pledged and Fully pledged) and request for funding should be developed, harmonised and standardised and provision should be inbuilt for linking activities to the funding work such that discrepancies be easily detected and remedial cure undertaken immediately without delay;
 
e) All contributions(financial and other resources) provided by these organisations be costed and consolidated into GF funding totals;
 
f) Negotiation be initiated with organisations enforcing stringent procedures which affect project implementation planning. Arrangements which suit Gf should be negotiated as fas as is feasible;
 
g) The key objective of Gf information systems should be developed to support planning and control of operations at every stage of each GF project's life cycle from inception through to completion. MIS which is required to achieve this should be timely, accurate, meaningful and relevant. The information needs of these organisations are clearly varied and have different purposes. However, the common goal of all participiants is to be able to plan, coordinate and control an array of prospective and live projects for monitoring them. Within this framework, each user's specific information needs should be identified and systems developed to achieve these requirements. GF information requirements be driven by a sectoral activity which can be carried out in any of the organisations. By standard caterorisation of each element of information which is required to plan, co-ordinate and control GF activities, information could be held across all GF projects within a standard database coding structures which would provide ease of consolidation and analysis of performance and hence improved management control and accountability;
 
h) Partner contributions should not be linked to their core business i.e a company that specialised in manifucturing of drugs for HIV should not contribute towards auxilliary activities such as supply of vehicles.
 
i) Salaries, drug trials and experimentations should not be funded under the partner contributions; and
 
j) Professional and consulting services of fees should not be paid to partners contribution in order to overcome tendency leading towards conflict of interest.

Global Fund is a financing organization that distributes funds  to countries according to action plan and should perform its role in a way that avoid all forms of conflict of interest while working with other partners .

From my long  experience  working in vaccine regulatory authority within drug regulatory authority , I would recommend that Global Fund procured Medicinal Drug Products against HIV/AIDS, T.B and Malaria should follow the same procedures used in UNICEF procured Vaccines donated to countries  to  be of assured quality, safety and efficacy from WHO Prequalified Manufacturers (public or private) that are manufactured under cGMP conditions and to be tested by independent competent regulatory authority in order to avoid all forms of conflict of interest.  GF should avoid donated pharmaceutical products that might not conform to international standards as public health concerns are of paramount importance as well as to avoid other donations from organizations that pursue specific religious , cultural or political causes that would question GF impartiality and might be considered as a form of propaganda to such entity that would have negative impact on the programme as a whole .

The same principle should be applied for clinical trials, GF should not participate in clinical trials utilizing medicinal pharmaceutical products as treatment  may succeed or   may not succeed.  Depending on the etiology and  specific disease prevalence  within a specific country; clinical trials are  performed by either  Industry- sponsored , Institutional- sponsored , Government- sponsored or Investigator-initiated clinical research. Guidance on various aspect of clinical research are available from WHO, ICH, CIOSM EMEA, FDA that should be implemented. International Ethics Guidelines should be followed , research is to be  described in protocol, risk identification , benefit risk assessment, review by independent Ethics Committee/IRB,  protocol compliance, informed consent, continued review/ongoing risk benefit assessment , Investigator qualification , records , confidentiality ,privacy , GMP , QS . Data on recruitment, eligibility, informed consent, safety management and reporting should be available. Selection of team members should be based on properly qualified, trained and experienced investigators and study personnel provided that forms of conflict of interest should be avoided..that  the existence of a well functioning pharmacovigilance  system that can monitor adverse events effectively.

In spite of extensive efforts and money are directed towards treatment still there is a rising numbers of more infected people each year, countries should set targets for prevention programmes . In light of the concept that ' Prevention is better than cure ' ; Stopping the spread of malaria requires a combination of effective treatment , through use of effective drugs of assured quality, safety and efficacy as well as spraying households and the use of insecticide- impregnated nets to kill the carrier mosquitoes. As tuberculosis spreads through insanitary and crowded living conditions and through poor nutrition, thus its prevention is linked more closely to improving the socioeconomic conditions, whereas , preventing HIV/AIDS and other STIs is a more related matter to changing the life styles of those affected group of people with unsound relations and prohibit  the use of contaminated needles by intravenous drug  users or prevent  the use of unsafe blood from remunerated donors. Private sector can successfully participate in increasing  public awareness  especially among the youth and to  prevent unsanitary habits of smoking shisha  which might aid in spread of TB that could be a real problem if not properly handled, , training of  medical and paramedical staff ,  perform subsidized testing and counseling which require collaborative efforts from all parties i.e Governmental , Public  & Private sectors , NGOs as well as Religious people  among others..

Private sector can provide great support to GF in the form of assisting in carrying programme activities , consulting services and can secure long-term sustainability for education and strengthening health infrastructure .

The understanding of requirement of an organisation is one of the vital factor of GF contribution.Number of countries e.g. African & Asian countries have required GF specific products, goods& services.GF specific products development could give physical &psychological support in the form of products(Drugs,equipment, technical assistance,tangible goods(consumer goods favoured en vi- ronmental factors, sales activity for convenience products & goods) in less GDP& less market,industries, infrastructure countries,these developmental factors could give strategic focus, intervention & expansion of specific market opportunity in GF product demand sensitive& demand oriented countries.In the above development foundation,campaign,potential donors cannot enlarge their own structure in contribution ,standard of conduct could be applied in above organisational development approaches. A systematic organisational structure could deal with critical product dimensions i.e. complexity of product structure,cost& quality for fulfill sociological,psychological needs of affected population ,society& community in less market opportunity developing countries.Organisations should ensure products& goods quality i.e. utilization associated with purpose,reliability & specification, physiological& psychological human consideration,instruction(proper- use) to market& community structure for non functional quality of products,goods,& services. Accessibility of GF services in developing & less industrial country is critical factor for sustainability of GF services implementation, organisation could ensure appropriate GF services policies&principles stimulation,building community goodwill, faith& social expectation by providing products& goods quality with specific standrad of conduct. Organisational approaches could help in identification of products& goods defects,providing adequate analysis& promotional efforts, time framework for evaluation,cost benefit analysis,demand& viability of the GF products & goods. These organisational contribution could give realistic objectives beyond donor government that contribute financial & other resources to the GF.

- There must be an organized system of administration.
 
- There must be proper accountability and good governance.
 
- There must be well built organizational capacity.
 
- Free flow of information and communication.
 
- Proper monitoring and evaluation tools in place.

Thanks for inviting Tecom Foundation to contribute once again to the 2008 forum. From your question those who are responsible to contribute to the sucess Global Fund are the IMF, Wold Bank. The reason is that Wold Bank was instituted to promote welfare of the people in the whole continent therefore if there are global pandemic such as HIV/ADS, TUBERCULOSIS and MALARIA then it is the obligation of the World Bank to help save the whole continent from these pandemic. IMFand the world super powers should also contribute positively to the sucess of global fund programs. Churches should also not be left out because welfare of its members is the proud concern of them.

 Dear members,

There is a long list of in-kind-donations. I think GF should restrict the donations within a limited list. By that GF can deal properly with type of donation. GF is financing organasation and disburses funds according to approved action plans. If we want to approve in-kind donations, this means that the GF should change the strategies and methods of work. Receiving funds from nongovernmental organizations should be planned in a way to avoid conflict of interest. Private sector can have an important role in increasing the support to GF. I think this the matter of importance on which the GF should put more emphasis.

When we think about in-kind contributions, we can also think about private sector's contribution. There are pros and cons of allowing private sector to allow in-kind donations. Obviously, many private actors will be interested in making profit but it is well-known that private sector is much more efficient than any other sectors.
With regards to donations from donor countries, some of these countries may have political or economic interest and may influence the action of the Global Fund. Clear policies and guidelines from Strategic Advisory Group with necessary adaptation according to positive criticism will be a good way to control non-humanitarian interest. Promoting positive criticism, like invitation for community or humanitarian workers point of view in this kind of forum, will be critical for future policies and plans.

Dear Alastair and all:

I think there should be some standards, if there are non you might end up with some donations that would be useless, or too dificult to use, or give to other countries... Donations in the form of experts knowledge in any field (economics, public health, and so) can be very valuable, as long as there is a Country willing to recieve them, or in need of them, therefore I think that before accepting any kind of non-monetary donation, there should be a requierement list for all of the countries... more like a wish list, where the countries PRs could write down some of their requierements (that are not monetary) and the GF could see if there is a donnor willing to meet those requierements...
 
For example: Supose that Bolivia needs an expert in the field of education to work in implementing curricular activities in school programs that help you prevent HIV/AIDS transmision. Then maybe there is an organization that has experts in the field of education, so the GF can work as a mediator between both ends, and get a benefit for all of them, the country benefits from the expert, the donnor benefits by their contribuition, and also gets the feeling that they are really doing something, and even gets the experience advantage, and the GF benefits from both, the donation, and the result in the country.... This logic can also be applied to products, goods, etc. as long as they are needed they can be recieved by donnors.

The global should accept non cash donations. But the global fund should have principles in place so that the donations given do not undermine the goal of the global fund. People or groups who would give global fund donations but with aim of furthering their interests, such kind of support should not be accepted. If a person or group of persons wants to donate they should declare their intentions so that the business of serving people from burden of disease is not diverted. The global fund should not be champion of groups whose aims and objectives do not reflect global fund agenda. But there definitely work with everyone who works for the benefit of the people irrespective of their background.

private companies & potential donors,GF could interact with potential
donor's various form of foundation,campaign,corporate,national govt,
constitutionally formed organisation.
Potential donors should also support international monetary system,
understanding of international liquidity with contribution to GF,
provide exchange arrangement in settlement of international transactions .
In fund raised &spent,finance instrument ,GF should consider
international payment mechanism including suitable instrument of
payments,forreign exchange instrument, convertible monetary units
prevalent in countries& fund recipient country.
GF innovative finance should also understand international liquidity,
international capital movement for fulfill
recipient & sub recipient
medium of payment mechanism.
In above process ,ODA(official development assistance & IFT)could
give additional capacity development support to GF specific inter-
national funding mechanism for donors,GF & partners.
GF secretariat could frame specific standard of conduct for restriction of financial contribution,creation of suitable reserves
by using international capital movement,&correct maladjustment in
accessibility between private(pharma,corporate) companies& affected population,public sector inclusion for procurement of products&
goods.GF framework document in general principles has detailed
structure including finance instrument,financial resources,
proposals,& innovative financing.
These structures could consider specific standards of conduct for
international acceptability of GF principles,financing measures,
international business opportunity& elimination of foreign exchange
restriction prevalent in less GNP countries.

What do you actually mean by REQUIREMENTS?
 
In my own opinion or point of view, any interested organisation/organisations other than the donor governments can contribute financially and materially to the global fund. Or do you want other organisations who can donate to the global fund pass through the donor governments inorder to donate to the GB? Why not called/mark donations and materials from other organisation other than the donor governments in a seperate scheme which could be called:AUXCILLARY DONORS SCHEME. Within the auxillary scheme exist seperate priority programmes like public health education in relation to Hiv/aids, Tb and malaria.
 
Since the donor governments disburses huge sum  of money each year to the global fund to fight Tb, malaria and hiv/aids; it will not be wise to accept financial and other contributions  from external organisations in the same scheme. Accountability may pose a serious problem.It is better to avoid it.
